Before you make a claim

Check that your illness or condition meets the definition in the policy. Being diagnosed with a critical illness by your doctor doesn't mean you automatically qualify for a benefit payment.

Sometimes only severe cases are covered. And there may be limitations and exclusions. For example, you may need to wait out a moratorium, survival or qualifying period.

  1. Information

  2. Document­ation

    Fill out the Critical illness claim form and ask your doctor to complete the Physician's statement.

    Gather any other supporting documents required. Then send everything through our secure site or by mail (address listed on the form).

    Good to know

    You're responsible for any fees your doctor charges to complete the medical statement.

  3. Analysis

    Once we receive the required documents, we review your claim. Then we contact you to let you know if your claim is approved. We also establish the payment method, if necessary.

  4. Payment

    If your claim is approved, you receive the insurance amount specified in your policy.
